Owners and skipper fined after trawler gas death in Fraserburgh
The owners and skipper of a fishing trawler have been fined following the death of a man who was overcome by gas on board. William Ironside, 52, was one of five men who fell ill on the Sunbeam in Fraserburgh in August 2018. At Peterhead Sheriff Court the owners, Sunbeam Limited, were fined £220,000 while skipper James Duthie was ordered to pay £7,500. Duthie, 66, was also sentenced to 100 hours of unpaid work. Confined Space: Dangerous Gas Buildup in Fish Holds
Four crewmembers onboard a commercial fishing vessel noticed an unusual odor coming from the fish hold while conducting shrimping operations. While under the assumption that the odor was due to a leaking refrigerant line, the crew hauled in their nets and began transiting back to port to have their system inspected by a service technician. As they were packing the last haul of catch into bags, one of the crewmembers went down into the fish hold to retrieve more bags. The crewmember subsequently fell back into the hold as he was trying to exit the space. A second crewmember failed to recognize the potential hazard and rushed into the space to provide assistance; however, he was immediately overcome by the gas and also fell into the hold. >click to read< 13:14
USCG medevacs 2 crewmen from fishing vessel 3 miles off South Padre Island, Texas
Coast Guard Sector/Air Station Corpus Christi command center watchstanders received a call on VHF-FM channel 16 at 10:34 a.m. from the commercial fishing vessel F/V Miss Verna stating that two crewmen were unconscious after entering a confined freezer space. Watchstanders consulted with the duty flight surgeon, who recommended a medevac. A Coast Guard Station South Padre Island 45-foot Response Boat–Medium crew launched to assist. Once on scene, the boat crew worked with South Padre Island Fire Department personnel to remove the crew members from the confined space and transfer them to the RB–M. An engineer on a fishing trawler died after he was asphyxiated by gas that leaked into a refrigerated saltwater tank where he was working without any safety precautions, an investigation has found. Three other crew members were also “nearly overcome” when they tried to rescue second engineer William Ironside on the trawler Sunbeam in Fraserburgh on August 14 2018. A Marine Accident Investigation Branch (MAIB) report found the fatal accident happened because Mr Ironside entered the tank, which was an enclosed space, “without any of the safety precautions normally associated with such an activity”. >click to read< 09:29

A fatal accident on-board a fishing boat in an Aberdeenshire harbour nearly resulted in multiple deaths, an investigation has found. A report by the Marine Accident Investigation Branch (MAIB) has made safety recommendations after the “tragic accident” in Fraserburgh. William Ironside, 52, died, while several other men fell ill while working on the Sunbeam in August.
